Referral pathway
We accept referrals from local authority placement teams, health partners and the Commissioning Alliance. We specialise in young adult males aged 18–25 with learning disabilities, autism spectrum disorder (asd), behaviours that challenge, and severe but enduring mental health disorders.
Eligibility
- Male, aged 18–25
- Primary diagnosis of learning disability and/or autism
- Behaviours that challenge and/or complex mental health
- Package includes supported living with personal care
What we need
- Pen picture and recent risk assessment
- Support plan and MDT reports (e.g., CLDT/CMHT)
- Medication & behaviour support plans
- Funding confirmation and legal status
